LIKELY QUESTIONS

- How have you designed or improved sample receipt, labelling, chain-of-custody, storage and disposal processes in a laboratory?
- What controls would you implement to ensure sample traceability from client submission through testing, reporting and retention or disposal?
- Describe your experience developing and implementing QA policies, SOPs and operational procedures in a regulated laboratory environment.
- How would you verify that quotations, request forms, analytical methods and laboratory work performed are aligned before testing begins?
- What is your approach to protecting data integrity, security and audit trails within a Laboratory Information Management System (LIMS)?
- How have you managed regulatory inspections, internal audits, non-conformances, CAPA and compliance with laboratory standards?
- How would you manage competing priorities when urgent client samples, routine work, limited storage capacity and staff shortages occur simultaneously?
- Describe your experience preparing departmental budgets, procurement plans and workplans while maintaining quality and service delivery targets.

BEHAVIOURAL QUESTIONS

- Tell us about a time you identified a breakdown in sample traceability or chain of custody.
Model approach: Situation: A sample tracking gap created risk of misidentification, delayed testing or incomplete records. Task: Protect sample integrity, identify the root cause and prevent recurrence. Action: Quarantined affected samples, reconciled physical inventory against LIMS records, investigated process and training gaps, updated SOPs and introduced controls such as barcode scanning, second-person verification and exception reporting. Result: Restored traceability, closed the deviation through CAPA, reduced errors and improved audit readiness.

- Describe a time you handled a difficult client complaint about laboratory turnaround time, results or service quality.
Model approach: Situation: A client escalated a complaint involving delayed results, unclear requirements or dissatisfaction with service. Task: Resolve the issue professionally while protecting technical integrity and compliance. Action: Acknowledged the concern promptly, reviewed the request form, sample status, method requirements and communication history, provided factual updates, coordinated corrective actions and documented the complaint. Result: The client received a clear resolution, expectations were reset, and the team improved communication or workflow controls to avoid recurrence.

- Give an example of when you led a team through a significant process change.
Model approach: Situation: The department needed to adopt a new SOP, LIMS process, storage arrangement or compliance requirement. Task: Implement the change without disrupting sample flow or compromising quality. Action: Assessed impacts, involved key staff, developed a training and rollout plan, monitored competency, addressed resistance and reviewed performance indicators after implementation. Result: The change was adopted consistently, staff capability improved and the department achieved better compliance, turnaround time or data accuracy.

- Tell us about a time you had to make a difficult decision with limited budget or resources.
Model approach: Situation: Budget constraints affected equipment, storage capacity, staffing, consumables or system improvements. Task: Maintain critical quality and service operations while prioritising expenditure. Action: Conducted a risk-based prioritisation, separated essential compliance needs from deferrable spending, used workload data to justify procurement, negotiated with suppliers and tracked budget performance. Result: Critical operations continued, high-risk gaps were addressed and leadership had a transparent, defensible procurement and budget plan.

SMART QUESTIONS TO ASK

- What are the most significant quality, sample-management or client-service challenges the department is expected to address in the first 12 months?
- Which laboratory standards, regulatory requirements and accreditation expectations are most relevant to this role, and are there any upcoming audits or assessments?
- What LIMS or electronic data repositories are currently in use, and what are the main opportunities for improving data integrity, traceability and reporting?
- How is performance measured for sample management and client services, for example turnaround time, sample rejection rates, complaint closure, audit findings or client satisfaction?
- What level of authority will the Deputy Director have over departmental budgets, procurement priorities, SOP approval, staffing and corrective-action implementation?

RED FLAGS TO WATCH FOR

- Unclear ownership of quality decisions, especially if commercial or turnaround-time pressure is allowed to override sample integrity, method suitability or compliance requirements.
- Persistent references to manual records, missing LIMS controls, incomplete audit trails or recurring sample identification problems without a funded improvement plan.
- Vague answers about staffing levels, storage capacity, equipment maintenance, procurement delays or management support for CAPA and regulatory compliance.
